Consent to the processing of personal data

In accordance with Articles 23, 24 of the Constitution of the Russian Federation, Federal Law No. 152-FZ of July 27, 2006 “On Personal Data”, the Personal Data Processing Policy posted on the website at: https://forma.agency/ (hereinafter referred to as the website), I, the subject of personal data, hereinafter referred to as the User, by sending/providing information when filling out forms and questionnaires on the website (hereinafter, each of them and all together – the form, forms) and further when using the Website and/or its services, express full, unconditional and unequivocal Consent to the processing of my personal data (hereinafter referred to as the Consent) on the following terms:

1. Consent is issued to Individual Entrepreneur Artem Sergeevich Lazarev (TIN: 272389994971 OGRNIP: 314272334300038)

2. Consent is given for the processing of personal and other data specified by the User in the forms by filling in the relevant text fields and/or files attached to the forms, namely the following categories:

  • last name, first name, patronymic, email address, phone number, other information provided by the User of the site (including when filling out the application form), on the basis of which it is possible to identify him/her as the subject of personal data.
  • data that is automatically transmitted during browsing and when visiting the pages of the site: IP address, information from cookies, browser information, access time, address of the page visited, referrer (address of the previous page);
  • statistics about my IP addresses.

3. Consent is given for the processing of personal data for the purposes of:

  • providing access to the site, its content and/or services, to the functionality of the site/services, for site administration;
  • identification when sending applications and/or using the website services;
  • provision of services, processing of requests and applications;
  • establishment of feedback, including sending notifications and requests;
  • confirmation of the completeness of the personal data provided;
  • collection of statistics by the Operator;
  • improvement of the quality of the website and/or its service, ease of use and development of new services;

I confirm that I am aware and agree that for the above purposes, the Operator has the right to collect and use additional information related to me, received during the user’s access to the site, its content and/or services, or from third parties, and including data on technical means (including mobile devices) and methods of technological interaction with the site and/or its service (including the host IP address, the user’s operating system type, browser type, geographic location, provider data, etc.), on the user’s activity when using the site and/or its service, cookies, information about errors given to the user, about downloaded files, videos, tools, as well as other data obtained in accordance with the methods established by the Personal Data Processing Policy; manage statistical information related to the functioning of the site and/or its service, as well as user information for the purposes of organizing the functioning and technical support of the site and/or its service and fulfilling the terms of the legislation of the Russian Federation, and the Personal Data Processing Policy developed in accordance with it.

4. Consent is given for the processing of personal data in a mixed (automated and non-automated) manner.

In the process of processing personal data, the Operator has the right to: collect, record, systematize, accumulate, store, clarify (update, change), extract, use, transfer (disseminate, provide, access), depersonalize, block, delete, destroy the User’s personal data.5.

5. I hereby confirm that:

5.1. I consent to the processing of my personal data specified in paragraph 2 of this consent, by the persons specified in paragraph 1 of this consent, and also confirm that the Operator has explained the consequences of failure to provide personal data, in cases where the provision of such data is mandatory in accordance with federal law.

5.2. I have been informed of the possibility of revoking consent based on the provisions of the Federal Law of 27.07.2006 No. 152-FZ “On Personal Data” by personally contacting or sending a written request (including in the form of an electronic document signed with a simple electronic signature or an enhanced qualified electronic signature) addressed to the persons specified in paragraph 1 of the Consent, including I have been informed of the right to revoke Consent in order to terminate the processing by the person specified in paragraph 1 of the Consent of both all personal data specified in paragraph 2 of the Consent and separately biometric personal data (if such are requested by the Operator and provided by the User).

5.3. I have been informed that the persons specified in paragraph 1 of this Consent have the right to continue processing of the User’s personal data after receiving the revocation of Consent, as well as after the expiration of the Consent, if there are grounds provided for in Part 2 of Article 9 of the Federal Law of 27.07.2006 No. 152-FZ “On Personal Data”.

6. Consent comes into force from the moment it is sent and can be revoked by the User by sending a written statement in any form to the Operator by e-mail at info@forma.agency or by another means that allows to clearly determine the fact of its receipt by the addressee in accordance with the Personal Data Processing Policy.

7. Consent is valid for an indefinite period until it is revoked by the User. Consent ceases to be effective from the date specified in the User’s statement on the revocation of Consent to the processing of PD, but not earlier than the date following the date of actual receipt of the revocation of Consent by the Operator.

8. The date and time of formation, confirmation and sending of the Consent shall be considered the moment of marking the relevant field (or pressing the relevant button) in the Form and/or pressing the Form send button on any page of the Site and/or its Service.

9. Guided by Part 4 of Article 9 of the Federal Law of 27.07.2006 No. 152-FZ “On Personal Data”, I send it through a personal mobile device or personal computer by attaching it to a special form or by filling in the relevant fields of such form on the Site.

10. The User is informed that the Operator who issued the relevant order (consent) shall be responsible to him for the actions of the person processing his personal data on behalf of the Operator.
